State of Utah Requirements for all school aged children

7th Grade Booster Information update

Those claiming exemption from immunizations for personal or health reasons may obtain the proper forms by calling the Utah State Department of Health at 1-800-275-0659.

The Student Information System (SIS) is an online portal which allows parents to view their student's assignments, grades, etc. You will need login information for each student. Please contact NPA administrative staff if you do not have this information.

Troubleshooting tips for using SIS

• Turn off any pop-up blockers
• REQUIRES Internet Explorer 6.0 or higher to access SIS.  SIS should work with Mozilla Firefox
• Mac users:  you will need to download and use Firefox.
